Degree: High School diploma required; Associate's/Bachelor's Degree in Social Work or Human Services preferred

Experience: Experience with Runaway, Homeless and "at Risk Youth" or Adults preferred

Day Shift -

Location- City of Niagara Falls, NY


AGENCY REQUIREMENTS:

  • Valid NYS Driver's License
  • Use of personal vehicle during working hours as needed


AGENCY EXPECTATIONS:

  • Be available for supervisory consultation 24/7
  • Demonstrates knowledge of Program, Mission, and services of Pinnacle Community Services, Inc., (PCS)
  • Complies with all Policies and Procedures of Pinnacle Community Services, Inc., (PCS) (including NASW Code of Ethics, Collective Bargaining Agreement, and Corporate Compliance Policies) and all governmental and legal requirements
  • Attends and participates in appropriate Agency staff trainings and meetings
  • Advocates for cultural diversity/sensitivity as appropriate
  • Reports all suspected incidences of child abuse/neglect to NYS Hotline
  • Experience and knowledge on the use of computers, cell phones and a broad understanding of the Internet and e-mail in the workplace


JOB RESPONSIBILITIES:

  • Maintains regular contact with Street and Shelter participants
  • Assists participants with navigating the social services network to obtain and/or maintain benefits
  • Performs regular outreach throughout the community including to soup kitchens/pantries, shelters, encampments, and community organizations
  • Participates in weekend, night and/or early morning outreach with the Outreach Team as needed
  • Distributes Personal Protective Equipment (PPE) and other emergency items
  • Conducts and scores housing-related and other assessments in order to coordinate services
  • Collaborates with local agencies and case workers to build positive working and referral relationships
  • Keeps records of participant interactions in assigned data systems
  • Responsible for data collection and reporting as assigned by Supervisor
  • Communicates pertinent information regarding participant needs to other staff members as appropriate
  • Ensures participant confidentiality, HIPPA and other regulations as identified
  • Attends and participates in regular staff and community meetings including but not limited to Pinnacle Community Services, Inc. and Niagara County Coalition for Services to the Homeless
  • Attends seminars, trainings, and workshops as identified by Supervisor
  • Participates in community and Agency-wide special events
  • Performs other duties as requested by Supervisor


PHYSICAL/ENVIRONMENTAL DEMANDS:

  • Ability to work in a crisis intervention setting
  • Job performed in a modern office setting
  • Ability to sit, stand and walk through the day
  • Ability to push, pull, bend, lift or carry up to 20 pounds throughout the course of the day related to office equipment such as p.c., journals, and binders
  • The function of this position usually performed community engagement with youth and families throughout the day. Some amount of office work will be required, however a good amount of stooping, kneeling, bending, crouching, lifting, walking, carrying and other movements may be required. Tasks involving working on a computer keyboard, computer software applications, calculations, cutting, labeling and taping involve extensive wrist and hand movement. All individuals are required to be able to perform these movements without significant risk of injury of themselves or others.
