Job description

Essential Functions:

Must keep up-to-date and adhere to Maryland Department of Health Office of Health Care Quality (OHCQ) regulations and Sagepoint Living Services policies and procedures.

Tracks and records interaction with clients and caregivers and report clients progress health status from admission to discharge.

Follow-up with clients within 1st, 30th and 45th day(s) of service, and then on-going, and track admission/readmission to emergency room visit or hospital stay.

Coordinates scheduling of caregivers for clients under the supervision of the Supervising RN.

Coordinates on-site interview process for all potential caregiver candidates with the Home Care Office Manager

Ensures all clients and employee records are up to date

Transfers and disposes of records according to retention schedules and policies

Acts as a liaison between clients and caregivers when necessary and respond to concerns

Ability to keep both client and caregiver information confidential in accordance to HIPPA Regulations

Reviews Care Plan goals with clients both in person and over the phone.

Manages referrals when needed, report to appropriate agencies that assist the client in achieving the goals and objectives identified in their Care Plan.

Coordinates Home Visits to evaluate caregivers and client documentation ensuring proper care is being provided.

Works closely with other Home Care Coordinators and Home Care Office Manager on pre-visit planning, scheduling, staffing and client management.

14. Ensures compliance with the customer requirements, quality policy, company standards, regulatory and external standards.

15. Monitors quality indicators related to ensuring employees conform to Sagepoint Quality Assurance Program in the care of its clients.

16. Maintains and improves service to clients by investigating customer complaints; collaborating with other staff members and management and training methods.

Provides coverage on after-hours coverage (1) week per month

Other Duties:

1. Performs other duties as assigned which are supportive in nature to the essential functions of the job, but which may be altered or redesigned depending upon individual circumstances.
