We're seeking the right candidate to join us for a full-time position at Vanguard Medical Group. We offer innovative primary care across all sites of our Family Practice organization. The Population Health Specialist (PHS) is an essential member of our dynamic health care team, reporting to the local nurse manager, focused on supporting the activities of the Care Coordination Department. Although this isn?t a hands-on clinical position, the PHS must have a good working knowledge of the health care field. This is a full-time position with a compensation of $20.00 - $22.00 per hour.
Key Responsibilities:
- Provide resources and assistance to patients who require clinical, insurance-based, and supportive care services
- Facilitate patient appointments with Primary Care Provider, specialists, labs, and other healthcare entities; obtain referrals and insurance authorizations
- Make outreach calls to patients regarding Emergency Room visits; schedule follow-up appointments; educate patients about alternatives to Emergency Room usage
- Accurate data management including the retrieval of hospital records and using Excel spreadsheets for a variety of value-based programs
- Perform outreach activities in association with clinical and utilization metric management
- Participate in Population Health initiatives including managing outreach programs, mailings, and patient education programs
- Attend trainings, meetings and presentations associated with the Care Coordination Department and the VMG organization as required
- Perform administrative duties including copying, report collation, organizational tasks, and other duties as assigned by Director
