Greater Family Health is a multi-site federally qualified health center providing comprehensive medical, dental and behavioral health services within the Chicagoland area. Your Goal:
To assess, evaluate, educate, and provide intervention and referrals to facilitate coordination of medical health, oral health and social services for low-income families.

How You'll Help Patients:
  • Accurately documents client activity in Cornerstone.
  • Monitors and assesses client/family needs.
  • Provides Case Management in accordance with the standards of the Illinois Department of Human Services Family Case Management Program (IDHS FCM).
  • Schedules and performs weekly home visits for all at risk clients.
  • Performs Ages and Stages Questionnaires with applicable clients.
  • Reports to all scheduled work sites and shifts on time and fully prepared to engage in all job responsibilities; location of work sites and shifts will change at the discretion of the supervisor.
  • Engages in all offered training, consultations and supervisions made available. Fully and professionally respond to and implement directives and advice offered by all supervisory and training personnel, as well as professionally offered feedback from coworkers.

What You'll Need:
  • Graduate of an accredited school with a Bachelor of Science or Bachelor of Arts in Psychology, Sociology, Public Health, or related field is preferred. An Associate degree with relevant experience may be considered.
  • Excellent written and verbal communication skills.
  • Bilingual in Spanish Preferred.

This role's work location will be split between our Wheeling, IL & Palatine, IL locations.
