Job Title: Community Health Educator – Level 1Department: Ivy Center for Family Wellness
Reports To: Ivy Center for Family Wellness Manager
FLSA Status: Non-Exempt Job Summary: Assist and provide culturally aware nutrition education that promotes and supports healthy lifestyles for the low-income, underserved patients of St. Vincent de Paul with or are at risk for chronic diseases such as obesity, pre-diabetes, diabetes, and associated co-morbidities. Specific Duties: • Assist with implementation of individualized patient/client/customer interventions and education as assigned by department providers (I.e., Diabetes Care and Education Specialists (CDCES), Registered Dietitian Nutritionists (RDN)). • Participate in education programs and services including obtaining patient/client screening data, conducting or following up with patient/client-specific assigned tasks, and contributing to the nutrition intervention, monitoring, and evaluation to support and encourage patient adherence to care plans. • Use data collection tools, obtain health and nutrition-related data to support client assessment, identify care needs and health or nutrition education, and provide referrals to appropriate internal and external community resources to meet client-centered needs per ICFW protocol • Deliver education programs, with approval of and under the supervision of the RDN, to individuals and groups that include diabetes self-management (DSME), diabetes prevention education (DPP), workshops and outreach events that are held at on-campus and off-campus locations. • Prepare educational materials (ex: handouts, food preparation, etc.) for programs and services offered including consults, classes/workshops, recruitment events, food demos • Use evidence-based behavior change concepts and apply communication skills and strategies to assist patients/clients with sustainable lifestyle changes and effective management of chronic disease. • Assist in the creation and revision of educational materials that support program services and are appropriate for age, literacy level, cultural background, and physical and cognitive abilities of recipients. (ex. articles, flyers, handouts, educational boards, presentations, etc.) • Work with healthcare team to ensure complete documentation of services rendered through Electronic Health Record for patient visits as outlined in policies and procedures. • Work with community stakeholders and staff at ICFW to achieve program goals and continuous quality improvement. • Participate in outreach and promotion activities to expand the reach of services and increase the enrollment rate into educational offerings. • Work with ICFW staff and community stakeholders to achieve ICFW program goals and continuous quality improvement. • Provide own transportation for regular travel within the Metropolitan Phoenix Area. • Complete required organizational training, refresher or new skills training, and training offered by the ICFW. • Comply with all applicable laws and regulations, including those governing participant privacy and data security (e.g., the Health Insurance Portability and Accountability Act [HIPAA]) • Participate in and serve as a model for SVdP’s Culture of Philanthropy, and ensure SVdP volunteers feel welcomed and appreciated * Volunteer engagement and appreciation, as well as Culture of Philanthropy participation are part of all job descriptions at SVdP, and a very important part of the organizational culture and values • Specific duties are not limited to those listed. Must perform other duties as directed by the ICFW Director. Required Skills/Abilities: • Ability to relate to low income, minority individuals. Appreciation for diversity and culture. • Comprehensive teaching skills with diverse groups. • Demonstrate professional and respectful attitude with clients/participants and staff. • Effective listening, written and oral communication skills. Strong public speaking and presentation ability. • Organizational skills, interpersonal skills, problem-solving abilities, and collaborative leadership experiences. Flexibility, resiliency, and ability to operate with minimal guidance. • Basic skills in Word, Excel, and other Microsoft programs. • Bilingual; Spanish and English. • Arizona driver’s license, proof of vehicle insurance, and personal transportation. • Availability from Monday through Friday between AM and PM hours. Some availability may be required throughout the year on the weekends. Education and Experience: • A bachelor’s degree obtained or in progress in a healthcare related field. Dietetics/Nutrition (Preferred). • Less than 1 year of experience in diabetes prevention or management education and care. Physical Requirements: • Able to stand, walk, sit, bend, reach and lift up to 30 pounds are required for this position
