The Family Support Specialist position is designed to support children ages 5 through 18 and their families across

environments of home, school, and community focusing on adaptive skills, emotional regulation, and relationship

building. Provides clinical case management, care coordination, parenting education, and supportive behavioral counseling for an evolving caseload of clients. Works collaboratively from intake to discharge with internal clinicians and external multidisciplinary team (MDT) members to ensure continuity and consistency of care for clients and families. Provides clinical case support and recommendations to families as well as the MDT. Actively participates in individual supervision and other required meetings. Maintains up-to-date documentation and overall quality assurance for assigned clients.

This position will have a mixed caseload, serving both general community-based families and families in our BRANCH program, Behavior Resources and Adaptive Needs in the Community and Home, which serves children who meet criteria for a moderate functioning level and may, or may not, have a diagnosis of autism spectrum disorder paired with a mental health diagnosis. These services will be provided at 20 hours per week depending on need and intensity for an average of 6-24 months. The program will provide assessment of needs, skill building for clients and caregivers, case management and resourcing. Caregivers are highly involved in treatment through engaging in family therapy and skill building.


Essential Job Functions

  • Provide support, counseling, and interventions in the child’s home, school and/or other community locations that is focused on building children’s and caregiver’s competencies.
  • Form a positive therapeutic alliance with children and families. Provide behavior management consultation, parent or life-skills training, and general support based on empowerment, strength-based and solution-focused frameworks.
  • Maintain working knowledge of community resources; ranging from food and clothing to job training & recreational programs, aid and support to families enabling them to access community resources.
  • Provide behavior management consultation and regular on-going support to school classrooms. This includes individual support to students, behavioral consultation to teachers and school staff, classroom groups, crisis intervention, or specific services requested to assist students and families.
  • Provide clinical feedback to clinicians to assist with the development of the treatment plan goals and objectives.
  • Conduct sessions with clients and families based on the goals and objectives outlined in the treatment plan.
  • Form positive work alliances with representatives of community agencies including schools, mental health, social services, probation, and others.
  • Maintain open communication with internal treatment team members to ensure congruency in treatments provided.
  • Participate in any necessary case meetings including, IEP, re-entry or school transition meetings, case staffing or other requested meetings.
  • Meet weekly productivity goals.
  • Complete documentation of individual and family sessions, case management contacts, and other required forms of paperwork within required timeframes of Medicaid standards and other contracted referral sources.


Other Job Functions

  • Participate in team meetings, as well as individual and group supervision.
  • Assist clinician’s in completing intake assessments.
  • Provide transportation for children & families when therapeutically indicated.
  • Attend scheduled internal trainings and participate in continuing education and professional development to develop increasing levels of expertise, areas of specialization, and professional goals.
  • Maintain general understanding of educational paperwork and IEP law.
  • Maintain the highest standard of cultural competency in all dealings with clients and staff.
  • Demonstrate knowledge and ability to implement agency policies, procedures, SPB’s, collateral policies, and COA’s guidelines.

Physical Requirements

  • Must be able to sit for prolonged periods of time in an office setting.


Working Conditions

  • Working in the community, in client’s homes, and in schools.
  • Mobile Office provided: car, gas card, laptop, hotspot.
  • Telehealth sessions can be performed from a remote, confidential location when necessary.
  • Schedule is Sunday to Thursday or Tuesday to Saturday with evening hours and on weekdays.


Minimum Qualifications

  • B.A. in Human Services or related field preferred.
  • Prior experience (2-3 years preferred) with EBD children and their families.
  • Training and demonstrated skills in crisis intervention.
  • Strong theoretical background/understanding of clinical issues and family systems.
  • Must be flexible to schedule appointments when families are available; most appointments will occur in the evenings and on weekends.
  • Must have the ability to work independently, have strong communication/coordination skills, and make clinically sound decisions.
  • Experience and knowledge working with neuroatypical population, implementing behavioral interventions.
  • Valid CO driver’s license
