Overview
Responsible for oversight of long term care facilities pharmacy operations, pharmacy policy and procedure development as related to long term care facilities, and long term care facilities policy and procedure development related to pharmacy. Provides required oversight for pharmaceutical services and clinical pharmacy services provided to long term care facilities. Maximizes customer/resident quality of service within boundaries of expertise. Acts as a liaison between the facility and pharmacy services and performs all duties per state board regulations.

Riverside Health System is currently seeking an labor pool (as needed) Pharmacist Specialist LTC in Newport News, Virginia.
Requirements:
- BS or Pharm D from an accredited school of pharmacy (Required)
- 2 years Clinical pharmacy experience (Required)
- 1 year Gerontology or LTC experience (Required)
- Licensed Pharmacist - Virginia Department of Health Professions (Required)
- Current, valid drivers license and acceptable driving record required upon hire and to be maintained during employment. Must also have reliable transportation to travel to facilities (Required)

What you will do
- Monitors patient profiles to assure appropriate dosage and therapy. Provides current drug information.
- Assists with formulary/protocol development and management related to specialty area(s).
- Handles all aspects of pharmaceutical care within the assigned location including supervision of pharmacy technicians.
- Monitors patient progress, reconciles data discrepancies, handles specialty pharmacy inquiries, provides departmental updates, and promotes services to RMG practices. Provide statistical data related to specialty pharmacy capture rate and leakage of prescriptions from the RMG practice sites.
- Performs pharmacotherapeutic monitoring and provision of pharmaceutical care for patients in the area of service responsibility in a routine, continuous manner, taking into account the age and clinical condition of each patient.
- Provides general and patient specific drug information to patients, physicians, nurses and, and other health care professionals.
- Interacts routinely with physicians, nurses, and other health care professionals to provide optimal pharmaceutical care to patients.
- Supports and participates in pharmaceutical related research in area of specialty.
