Position: Clinical Care Coordinator, Logistics Center

  • Responsible for appropriate placements for patient admissions, coordinating and expediting admissions and transfers, and overcoming placement delays and obstacles.
  • Act as a primary liaison between all services and departments, and will work in partnership with physicians to ensure appropriate patient placement for effective, efficient patient focused outcomes.
  • Providing centralized one-call service for direct admissions and facility transfers.
  • Monitoring bed requests for post-procedure admissions, and assigning bed and status prior to admission.
  • Placing each patient and verifying he/she is in the appropriate level of care and correct patient status/type.
  • Using clinical knowledge to plan and coordinate hospital-wide patient placement activities.
  • Collaborating with the Staffing Coordinator to consider nursing staff capacity in planning patient placement.
  • Coordinating with the Unit C3, BH C3 and ED C3 to facilitate patient placement including all admissions, transfers and discharges on a 24/7 basis.
  • Ensuring a pre-cert/authorization/clear admission status order is obtained for admissions in absence of C3a.
  • Obtaining clinical information and completing orders for direct admissions and facility transfers.
  • Creating and tracking direct admission and hospital transfer cases and associated documentation/activities in Intake Logistics.
  • Anticipating and facilitating accommodations for patient’s care needs for patients being admitted to the nursing unit (patient requiring a bariatric bed, special equipment, interpreter svc, etc.).
  • Reviewing previous patient history and updating CL with key patient attributes (isolation, 30-day readmission, etc.).
  • Anticipating, identifying, and addressing barriers that may impede system-wide flow, escalating issues as appropriate.
  • Leveraging information to adjust resources (EVS, Transport, Ancillary Services, & Staffing) in real-time to meet milestones and achieve length of stay goals.
  • Assisting Logistics C3a with priorities regarding patient flow to ancillary testing and procedure areas.
  • Serving as a consultant and educator with medical staff regarding patient status, bed placement, payer requirements, and utilization of resources.
  • Managing the Logistic Center staffing process for assigned shifts, as needed.
  • Meeting deadlines and exceeding metrics identified for the Logistics Center through attention to detail and implementation of facility policies and Standard Operating Procedures.
  • Following, analyzing and reporting patient placement issues, coordinating network SIN reporting to identify causes and trends.
  • Working collaboratively to solve problems, improve processes, and integrate services within the department, with other departments and throughout the network.
  • Making frequent rounds in all patient care areas and ensuring frequent communication with clinics and affiliates to communicate with staff regarding patient placement and transfers.
Qualifications:
Job Requirements:

Bachelor’s degree or foreign equivalent in Nursing or a related healthcare field.

Must have valid New York State Registered Professional Nurse license.

1st, 2nd, 3rd shifts and weekends as business needs require.
