Central Support Pharmacist

The role of the Central Support Clinical Pharmacist is to own the clinical outcomes for each patient that we serve. To ensure the best clinical outcomes, this person should also be prepared to assist with non-clinical tasks that their patients require, including prior authorizations, or financial assistance, among others. The Clinical Pharmacist will also be responsible for developing and implementing evidence based clinical protocols, as well as driving clinical quality improvement at the customer site. This person will also be responsible for supporting clinical work done at client sites across the country.

This work will be done remotely and on-site. The chosen candidate must have the ability to travel up to 50% of the time and must be available to work PST and/ or HST hours depending on business needs.

  • Leading work in various clinic(s), owning clinical outcome KPIs (as set) and clinical dashboards, and ensuring that the patient/Trellis engagement is appropriate and consistent with customer preferences. This includes:
    • When operating in clinic: Ensuring that every patient who can benefit from specialty pharmacy services is identified and either met in clinic upon arrival.
    • When operating in Central Operations: Ensuring that every patient who can benefit from specialty pharmacy services is identified and contacted by phone or other means of verbal or text communication.
    • Implementing an engagement that highlights the benefits of using the customer’s specialty pharmacy services, without impeding the patient choice of pharmacy
    • Leading clinical follow-up with patients per defined protocols – through outbound phone calls and in-person appointments – to ensure patients are adherent to therapy and any issues are identified and resolved expediently
    • Accepting inbound calls from patients, and sharing in on-call coverage to ensure 24/7 patient support
    • Appropriately addressing clinical questions, triaging as appropriate
  • Partnering with physicians in clinics to ensure therapies selected are evidence-based
  • Participating in leadership and clinic staff meetings to help explain the Trellis model
  • Assisting the training team with training of Pharmacy Liaisons and Clinical Pharmacy Specialists
  • Assisting our Quality and Accreditation team with compliance related tasks to ensure we promote a culture of compliance and deliver a program dedicated to improving patient safety
  • Assisting our General Managers, across our customer sites, with providing direct patient care to support continuous implementation of expanded services. This may require travel to customer sites to cover clinician PTO, to support the launch of new service lines, or to provide additional clinical services
  • Support our VP of Clinical Affairs by providing additional clinical bandwidth for outcome initiatives, both remotely and at customer sites
  • Assist the Advance/Implementations Team in new clinic launches
  • Establishing great working relationships with clinic staff and physicians in customer clinics

Oversight responsibilities:

  • Developing and implementing best-in-class clinical and research protocols for specific disease states, to markedly improve patient outcomes
  • Assessing and developing medication management plans for patients while ensuring appropriateness of therapy
  • Ensuring that medications are cost-effective and affordable by the patient
  • Owning clinical quality improvement to drive key clinical outcome indicators at a patient and population level
  • Developing training materials to help onboard Ambulatory Clinical Specialists and on-site Pharmacy Liaisons
  • Developing and delivering workshops and development seminars for client-based pharmacy specialists and leadership groups
  • Representing Trellis at conferences and workshops
  • Assisting in or developing abstracts and presenting posters at national conferences as warranted.
  • Medical writing to include development of white papers and other clinical content to be published in peer reviewed journals

Qualifications:

  • Advanced degree or Board Certification (e.g., PharmD, M.S., Board Certification in a subspecialty)
  • Licensed to practice, or the ability to become licensed as a pharmacist in the states where Trellis sites are located
  • The energy and willingness to be hands-on and do whatever is required to provide the best care to our patients
  • The ability to start from scratch, execute quickly and accurately without pre-defined processes and protocols, and an interest in defining such processes and protocols for the future
  • Strong people skills and the ability to work with teams across various clinics and health system partners
  • The ability to communicate effectively at all levels inside and outside the organization
  • Proficiency in MS Office including Word, Excel, PowerPoint, and Outlook Calendar

And bonus points if you have:

  • Experience as an Ambulatory Clinical Specialist in a specialty disease setting and the ability to work across multiple specialties
  • Proficiency in multiple EMR systems such as EPIC, Cerner or Meditech
  • Proficiency with dispensing systems such as QS1, RX30, Computer Rx etc. along with experience in pharmacy dispensing
  • PGY2 Residency in Specialty Pharmacy

  • (PPD) TB Skin Test - Proof of negative TB skin test within the last 12 months
  • (MMR) Measles, Mumps and Rubella or A Blood Titer proving immunity

  • Hep B3 Series
  • (Flu) Influenza-required for hire between Oct 1st-April 30th
