Position: Registered Nurse (Case Manager)

Description: Registered nurse (RN) case managers advise on and coordinate inpatient and/or outpatient treatment for patients, contingent on their needs. In this sense, RN case managers typically assume a less hands-on, though integral, role in patients' treatment. Registered nurse case managers are responsible for coordinating patient care services. They help determine the type of care needed and the steps that must be taken to ensure that the patient receives that care. Other responsibilities can include scheduling appointments, determining insurance eligibility for certain treatments and performing exams and evaluations.

Location: Pittsfield, MA 01201

Shift: Monday-Friday 5x8 hours Day Shifts (07:00 - 15:30)

Guaranteed Hours: 40 hours per week

Job Type: 13 weeks contract job with possible extensions

Requirements:

• Active RN state license
• BLS certification from AHA
• Three years clinical experience in acute care in areas such as internal medicine, surgery, orthopedics, mother/child, cardiology, and oncology.
• Three years clinical experience in Rehab Setting, Home Health and Long-Term Care would be considered.
• Ability to perform a comprehensive admission and discharge risk assessment related to quality, financial, payer benefit allowances and limitations, risk management and patient satisfaction.
• ACM or CCM Certification is a plus
