Position Summary:

Reporting to the NCIU Manager and Medical Director, the Case Manager will serve as the primary person to coordinate neonatal High Risk Infant Follow-up (HRIF) services among the county CCS programs, HRIF programs located in community and intermediate NICUs, state CMS regional offices, clients/families, and others related to HRIF services. Ensures client eligibility for the HRIF program and coordinates services with State and County CCS Programs and other local agencies. Functions as the key person responsible for the data collation reporting for patients in the HRIF program and works closely with affiliate hospitals and the HRIF team in meeting mandated reporting requirements. Works with the NICU Medical Director to ensure the HRIF program fully participates in the CMS Branch Program evaluation process by providing required data and information and participates in the CMS Branch in developing quality improvement (QI) activities including the development of performance measurement and required QI reports. Participates in NICU discharge planning and actively ensures client referrals and services including follow up. Gathers medical reports and assessments for review by team members and incorporates the psycho-motor and age-appropriate cognitive skills of the patient/family utilizing knowledge of patient assessment, planning, intervention and evaluation. Travels as needed to affiliate hospitals to support HRIF program and duties.

Salary Range:
Annual Rate Minimum $121,709.52 Midpoint $141,169.68 Maximum $160,608.96
Required Qualifications:
Bachelor of Science in Nursing
Current CA RN license
Ability to maintain a work pace appropriate to the workload
Must demonstrate customer service skills appropriate to the job
Excellent written and verbal communication skills in English
Basic Life Support Certification from AHA
Must possess the skill, knowledge and ability essential to the successful performance of assigned duties
Minimum 2 years of clinical experience in a Neonatal ICU
Certifications as required by affiliate hospitals

Preferred Qualifications:
Understanding of TJC requirements
Bilingual skills
Knowledge of third-party payers including Medi-Cal/ Cal-Optima and CCS.
One year of experience in a HRIF program or as a discharge planner for an NICU or a community based Medically Vulnerable Infant Program.
Knowledge of CCS Center Team Regulations.
