Job description
In collaboration with our community Partners, both on and off-site, the Case Manager provides intensive housing-focused services to Gateway clients, frequently meeting and engaging with them in furtherance of obtaining appropriate and affordable housing or program in the community as quickly as possible. This individual aids guests in creating a dynamic case plan leading to obtaining and maintaining a safe, permanent residence and promotes the guest’s stability by providing a wide range of services and community resources. The Case Manager works as a member of an interdisciplinary team to assist clients in achieving the highest goals possible. This individual should have a background in Chemical Dependency, Mental Health Case Management and Counseling. This position is responsible for assisting the Rehousing Manager in monitoring the performance of any Case Manager Interns.
