InterCommunity, Inc is a FQHC LA (Federally Qualified Health Center Look Alike). Our health center provides same-day primary care services to every member of the community, regardless of life situation or ability to pay. We also provide a wide array of behavioral health services. Our community Health Care Centers are located at 281 Main St. East Hartford, 16 Coventry St. Hartford and 828 Sullivan Ave., South Windsor. InterCommunity’s Addiction Services offers a similarly wide range of substance use treatment and recovery-supportive services, with many sites in the Hartford area. We provide services across the spectrum of care, including primary care, residential detoxification and substance use treatment, multiple levels of residential care, outpatient substance use disorder and mental health services for adults and children, intensive outpatient programs, employment and community support, mobile crisis evaluations, judicial support services and social rehabilitation.

Summary:
The Care Coordinator assists all patients through the system by acting as a patient advocate and navigator.
Essential Duties & Responsibilities:
- Meets with all clients to ensure appropriate insurance coverage. Gathers all necessary information for insurance activation.
- Verifies insurance coverage and benefits.
- Assist clients in taking all steps necessary for them to have benefits.
- Ensures all treatment authorizations are in place.
- Communicates with Beacon Health Options, Department of Social Services and any other provider to ensure treatment authorizations.
- Facilitates claim appeal process to obtain insurance authorizations.
- Works collaboratively with the treatment team to ensure best client outcome.
- May participate as a member of an interdisciplinary treatment team in the assessment, development, implementation and evaluation of client treatment or health/mental health care plans.
- Collect necessary data to be reported to DMHAS related to client health information and outcomes.
- Performs related duties as required
Education &/Or Experience:
High School Diploma and two years of experience in healthcare field, Bachelor’s Degree preferred.
