We have an incredible opportunity for a Care Coordinator at several of our NY locations. Level will be commensurate based upon years of relevant work experience and education.
As a Care Coordinator you will be responsible for the delivery of quality psycho-social care to the youth and families that we serve. This position works with Clinicians and other care providers to ensure that overall needs of the client are met.
Responsibilities :

  • Work as a team member in assessing a youth's needs and is responsible for developing, assessing and modifying the individual service plan in collaboration with the child/youth/family, family and multi-disciplinary treatment team
  • When appropriate, provides psycho-educational training for youth and families in a variety of settings based on group/child/youth/family needs and Care Coordinator experience
  • Actively involved in the implementation of a strength-based service plan including site/home visits and assessments, identifying and coordinating collateral resources, recognizing gaps and needs, assessing results and communicating with the multi-disciplinary treatment team
  • Participate in agency and team meetings, volunteering and serving on agency and community committees if needed.
  • Mentor Care Coordinator I's and other staff if needed
  • Utilizes experience, education, age-specific knowledge and cultural competence in order to assess and support youth and families to achieve positive and successful outcomes and routinely successfully uses problem-solving skills with self and others

Requirements :

  • Master's degree in the human services field and 2 years of experience in providing direct services to or in linking children with serious emotional disturbance to services
  • Or a Bachelor's degree in human services field and four years’ experience in providing direct services to or linking children with serious emotional disturbance to a broad array of services
  • Must have a valid New York State Driver's License and meet agency driving and insurance standards
