ph1JOB SUMMARY: /h1p /ppAn individual who, under the direction of a Registered Nurse, provides personal care services within the guidelines of the Interdisciplinary Group Plan of Care to Patients enrolled in the Hospice Program./pp /ppstronguQUALIFICATIONS: /u/strong/ppstrongu /u/strong/pol start='1' type='1'liMeets federal Hospice Aide training and competency requirements. /li/olp /pol start='2' type='1'liMust be certified by the state of Arizona./li/olp /pol start='3' type='1'liSpeaks, writes, reads, and comprehends English/li/olp /pol start='4' type='1'liAble to lift, push, and pull light to moderate loads./li/olp /ppstronguRESPONSIBILITIES /u/strongstronguincluding but not limited to/u/strongstrongu: /u/strong/ppstrongu /u/strong/pol start='1' type='1'liUnder the direction and ongoing supervision of the RN, provides essential personal care assistance to the client/li/olp /pol start='2' type='1'liPreparing and serving meals according to POC./li/olp /pol start='3' type='1'liAssist with bath, care of mouth, skin and hair./li/olp /pol start='4' type='1'liAssist with prescribed exercises, which the patient and Hospice Aide have been taught by appropriate professional personnel./li/olp /pol start='5' type='1'liBy performing such incidental household services as are essential to the patient’s well-being at home./li/olp /pol start='6' type='1'liTakes client’s temperature, pulse and respiration if requested by the POC./li/olp /pol start='7' type='1'liProvides emotional support and compassionate care to the client and family./li/olp /pol start='8' type='1'liMaintains required competency and skills./li/olp /pol start='9' type='1'liReports any changes in the patient’s condition or family situation immediately by calling the Hospice office and notifying the Team Leader and/or the Patient Care Coordinator./li/olp /pol start='10' type='1'liPractices Universal precautions according to OSHA regulations and Hospice Policy and Procedures./li/olp /pol start='11' type='1'liAttends at least twelve 12 hours of continuing education programs or approved audiovisuals per calendar year./li/olp /pol start='12' type='1'liDocuments all patient-related activities in a timely manner./li/olp /pol start='13' type='1'liReacts to change productively and performs other job-related tasks and duties as assigned./li/olp /ph3PHYSICAL DEMANDS: /h3p /ppAble to lift, push, and pull light to moderate loads.
