GENERAL DESCRIPTION:
Provides nutritional services in accordance with Day Program regulations. Supervises the nutritional needs of program participant in accordance with the recommended allowances of the Food and Nutrition Board of the National Research Council and National Academy of Science. Performs related work as requested.
ILLUSTRATIVE EXAMPLES OF WORK:
· Confers with AHRC physicians and other interdisciplinary team members concerning nutritional needs of program individuals we support.
· Prepares nutritional assessments of each individual on an annual basis.
· Provides dietary counseling as indicated in each Day Program individual’s service plan.
· Instructs individuals families and staff on the requirements and importance of nutritional needs.
· Monitors and supervises modified diets, prescribed by the physician. Keeps on file prescription of such modified diets.
· Prepares reports and maintains accurate monthly and quarterly records.
· Reviews physician prescribed diets no less than monthly.
REQUIRED KNOWLEDGE, SKILLS, ABILITIES:
· Good knowledge of nutritional principles and practices.
· Good knowledge of menu planning.
· Ability to establish and maintain effective working relationships in a manner conducive to full performance and high morale.
MINIMUM REQUIREMENTS
Must be registered by the American Dietetic Association and have a Bachelor's Degree in food and nutrition. NYS Licensed in Nutrition. Readiness to learn and utilize relevant agency computer applications.
NECESSARY SPECIAL REQUIREMENT:
As part of an ongoing agency initiative, AHRC Suffolk has partnered with CQL to perform Personal Outcome conversations with the men and women we support. As part of this initiative, all staff may be required to complete training in order to contribute as interviewers, as follow-up staff, and/or as interpreters when interviews are being conducted.
Candidate must possess and maintain a valid New York State Driver's License for the duration of employment.
Documentation of a current Mantoux tuberculosis test that is within 12 months of beginning employment and a second step PPD within three (3) weeks of beginning employment must be provided.

Pay: $80,000.00 - $85,000.00 per year
